Funding is available for up to 20 non-profit or community-based organizations who:
- Are located in and serving New York State (excluding NYC) and/or the Nations (excluding NYC)
- Are not hospitals or county health departments
- Have an operating budget of less than $1 million. The selected applicant must be the recipient of this contract (cannot use an umbrella organization or fiscal agent to receive contract).
AND
- Have not been previously awarded under this CDC COVID-19 Health Disparities grant OT21-2103

The Bureau of Health Equity and Community Engagement is seeking applications for 20 community-based organizations to conduct community wellness project and serve as credible messengers of COVID-19 public health information. Funding amount is up to $47,946 per organization through a deliverable-based contract.
This is a one-time only funding opportunity. Contracts will be from June 1, 2025 – January 30, 2026. Up to 20 awards will be available. Organizations can only be selected for one award.
This project is supported by funding from the CDC COVID-19 Health Disparities Grant OT21-2103.
