The HRI-50 Project Approval Form was modified in October 2017 to allow Adobe Reader users to save data typed into the form. Please use the new form in the future.
In order to save the file with data, please select ‘File,’ then ‘Save as’ and select a location to save the file locally.
The updated form is posted on the Office of Sponsored Programs internal forms site, https://internal.healthresearch.org/office-sponsored-programs/elements-of-a-full-application/forms/.
