In an effort to improve efficiency and enhance reporting capabilities, Health Research, Inc. has developed a new Project Approval Form (HRI-50). Beginning April 1, 2015, HRI will require that all applications be accompanied by the new HRI-50.
Key elements of the revised HRI-50 include a project abstract, a collaboration section, a disclosure of significant financial interest (FCOI) section, and an e-mail option. The Principal Investigator will no longer be required to complete a separate Financial Disclosure form; however, separate FCOI forms may be required for additional staff contributing to research projects. The Project Abstract field should contain a lay abstract, up to 250 words, that describes the scope of work. The collaboration section should include any significant collaborations with outside institutions or individuals. The email option allows the PI to insert the completed HRI-50 into a new e-mail. *This option will utilize the default e-mail software on a device.
HRI encourages PIs to submit the new form electronically via e-mail. Please note that images of electronic signatures are acceptable.
