The IRB has updated the Protocol Review Request Application Form (DOH 1871), Continuing Review Application, and the Study Closure Form.
NOTE ON STUDY CLOSURE: Please be aware that study closures should occur at the time the study closes (not at continuing review or in the follow-up report).
The most current research forms can be found on IRBNet under the Templates and Forms page (free registration on IRBNet required). Always check this page before submitting any documentation. Do not use forms from previous protocol submissions, as IRB Administration will not be able to accept the older versions.
